COVID-19 vaccination is now open to all healthcare workers (HCWs) in Phase 1A: All HCWs who have the potential for direct or indirect exposure to patients or infectious materials.

  • Healthcare worker employee badge with photo

  • Professional license AND a photo ID

  • Signed letter from employer on facility letterhead AND a photo ID

  • Payment stub from healthcare provider with your name AND a photo ID
